Select a client whom you observed or counseled this week. Then, address the following in your Practicum Journal: • Describe the client (without violating HIPAA regulations) and identify any pertinent history or medical information, including prescribed medications. • Using the DSM-5, explain and justify your diagnosis for this client. • Explain whether motivational interviewing would be effective with this client. Include expected outcomes based on this therapeutic approach. Support your approach with evidence-based literature. • Explain any legal and/or ethical implications related to counseling this client. PATIENT INFORMATION: (Please add information needed to complete the journal entry order) Patient is a 66-year-old female with a diagnosis of Anxiety disorder and Major depressive disorder. Narcotic pain medication seeker and endorses pain leading to depression symptoms. Regular amphetamines to offset sedation. Alert and oriented x 3, mood approachable, appears stated age, cooperative, speech normal. No suicidal ideation, plan or intent is noted or reported. Currently being seen by a psychologist and the psychiatrist for psychotherapy for stabilization of depressed moods, stabilization of anxiety, stabilization of irritability/anger and increase appropriate expression of angry feelings. A patent is engaged in the session. Progress is guarded as the patient appears to be struggling with psychosocial stressors, as well as adjustment to physical limitations and nursing home placement, which is exacerbating mood symptoms. Will continue with psychotherapy to increase adjustment to a medical condition/functional limitation and skilled nursing facility placement.
