Present once prevention effort that could have been used with this family.Susan, a toddler aged 1 ½, is brought into the emergency room by her mother, who has recently divorced from her husband and is unemployed. Susan has an injured arm, which her mother says she got earlier today while playing outdoors. Examination reveals a spiral fracture as well as some lacerations of the lip and eye. There are numerous quarter-sized bruises on both her arms and legs in various stages of healing. In an interview, the mother reveals that Susan has been walking since 11 months and that she “gets into everything.” The mother states that Susan hasn’t had any unusual problems with her health. She explains the bruises are the result of Susan’s falling and bumping into objects around the house and complains about this very much. The present injury, she says was the result of Susan’s falling off a picnic table in the backyard several hours ago. She says the eye and lip lacerations were obtained in the same accident. A check of hospital records indicates that Susan was seen in the emergency room six months ago for bronchitis, at which time it was noted that she was listless and under-weight. There was no history of pediatric care at that point, so Susan was assigned to the hospital clinic for routine checkups and inoculations. Clinic appointments had not been kept. Susan remains quiet during the examination and attempts to stifle her cries when she feels pain. Her mother offers no comfort and Susan seeks none from her, although she cuddles up to you when you speak soothingly to her.
