The Final Capstone Project Paper
Students will submit a report that summarizes the results of a community health assessment utilizing the social-ecological model to examine the social determinants of health of UAB SHARP patients. This paper will be supported by scholarly evidence and will demonstrate the student’s acquisition of graduate-level public health concentration knowledge and skills in addressing health disparities and the social determinants of health. You should tailor your report for UAB leadership in public health and social service as well as medicine and nursing. In your report, you will also demonstrate how your public health concentration competencies can be useful for key stakeholders in the community health assessments process

- Background Section (1.5-3 pages, 375-750 words)
- A brief overview of SHARP activities and the IP team. Describe how your team worked together to assess the needs of your patient as well as the methods used to investigate the social determinants of health.
- SDOH/SEM Assessment (3-5 pages, 750-1250 words)
- Individual-level contributing factors such as race, gender, knowledge, attitudes, and beliefs
- Interpersonal-level contributing factors such as family, peers, and social networks
- Organizational-level contributing factors such as the education system and employers
- Community-level contributing factors such as advocacy organizations and community coalitions
- Policy-level factors such as national, state, and local laws
- Interventions (2-3 pages, 500-750 words)
- Propose and prioritize two to three realistic public health actions, interventions, programs, or policies that you recommend for implementation that will improve the health of this patient and their community. Provide evidence of effectiveness from prior research.

- Describe how your public health concentration competencies can be useful for key stakeholders in the community health assessments process
